2009-2010 Hockey Season

WHAT YOU NEED TO BRING:
- Completed Pre-Printed Registration Form
- Check or Money
- Proof of South Stormont Residency: We will only accept the following (a copy of your residential phone bill, rent receipt or your heating bill) with your completed registration form and check(s). Note: We are not interested in the amounts on your bills please black out any financial information
Registration Dates:
- Saturday February 28th – 8:00 a.m.- 1:00 p.m.
- Sunday March 1 – 12:00 p.m. – 6:00 p.m.
- Friday March 6th – 5:00 p.m. – 8:00 p.m.
- Saturday March 7th – 8:00 a.m. – 1:00 p.m
For any new registrations or those of you who have moved during this season please contact the registrar – Kim Height (613) 936-2344 for your registration form. Note the following information is required
- Proof of residency
- Player Declaration of Residence You can find these forms at wwww.odha.com
AFTER AUGUST 1ST, 2009 (September 1, 2009 for new residence) We will be happy to place your child on our wait list.
NOTE: ANYONE REGISTERING AND/OR PAYMENTS RECEIVED AFTER AUGUST 1ST, 2009 WILL NOT BE PERMITTED TO TRY OUT FOR ANY COMPETITIVE PLAY (ALL LEVELS – REP. OR B HOUSE HOCKEY) UNLESS THE LATE FEE OF $100.00 IS PAID 24 HOURS PRIOR TO THE TRY OUT TIME AND THERE IS NO WAIT LIST FOR THE DIVISION THE PLAYER IS REGISTERING IN– Extension until September 1, 2009 for new residents of South Stormont (moved to the area between June 1st and August 30th, 2009)
If your child is trying out in any other association you must contact the registrar to inform her of the outcome of this try out. If you do not do so by September 15th, 2009 (EXEMPTION TO SEPTEMBER 30, 09 FOR MIDGET LEVEL ONLY) your registration check will be cashed and an administration fee of $25.00 will be deducted from your refund check.
